Positionnement des traitements de maintenance dans le cancer ovarien en première ligne en 2026

Introduction

Vous rencontrez en consultation Mme O. 74 ans, sans antécédent personnel ou familial. Devant des douleurs abdominales persistantes, cette patiente a bénéficié d'un scanner thoraco-abdomino-pelvien avec injection qui retrouve une masse ovarienne droite mixte tissulaire et multiloculaire suspecte, une atteinte ganglionnaire rétropéritonéale lombo-aortique, de multiples nodules péritonéaux et un épanchement péritonéal.

  • Le CA 125 est à 715 UI/ml.
  • La coelioscopie retrouve un stade FIGO IIIC, non résécable d'emblée, avec un Index de carcinose péritonéale (PCI) à 30.
  • Les biopsies réalisées sont en faveur d'un carcinome séreux de haut grade (CSHG) d'origine ovarienne.
  • Après validation en réunion de concertation pluridisciplinaire (RCP), devant le caractère inopérable d'emblée, la patiente a bénéficié d'une chimiothérapie première par Carboplatine AUC 5 et Paclitaxel 175 mg/m2 toutes les 3 semaines. Après 3 cycles, il existe une réponse morphologique partielle avec disparition de l'épanchement péritonéal et des adénopathies lombo-aortiques ; le Ca125 est à 158U/mL avec un score KELIMTM à 0.73.
  • Il est réalisé une chirurgie complète CC0 type ovaire sans complication au décours.
  • L'analyse anatomopathologique retrouve des localisations du CSHG connu avec un score de réponse à la chimiothérapie (CRS) à 2, soit une réponse tumorale partielle. Il est retrouvé une mutation somatique c.2809C>T du gène BRCA 2, et un statut HRD.
